Waiver & Permissions

Please read and sign the waiver below. All participants under 18 require a parent or guardian signature.

Participation Waiver — The Foundation Program

I, the undersigned parent or legal guardian, give permission for my child to participate in The Foundation, a youth strength and wellness program offered by The WELLness Collective, coached by Briana Weber.


I understand and agree that:


1. Physical activity involves inherent risks. I acknowledge that participation in strength training, conditioning, and movement activities carries the possibility of physical injury.


2. I certify that my child is physically able to participate in athletic activities and has no medical conditions that would prevent participation, except as disclosed in this form.


3. I authorize Briana Weber and The WELLness Collective to obtain emergency medical treatment for my child if I cannot be reached in an emergency.


4. I give permission for photos or videos taken during sessions to be used by The WELLness Collective for promotional purposes, including social media, unless I indicate otherwise below.


5. I release The WELLness Collective, Briana Weber, and any affiliated venues from liability for any injuries sustained during participation, provided reasonable safety standards are followed.


6. I agree to notify Briana Weber of any changes to my child's health or physical condition prior to sessions.
